Student Reading Log - Established Reader

The Student Reading Log - Established Reader helps you monitor student's daily reading and helps students become accountable for their own practice. This form is one of the resources available to help you implement Accelerated Reader in your classroom. This form can be found under Forms and Charts on the Resources page.

Your established readers can use this form to record the daily reading they do in and out of class. Teachers can use it to monitor their students' daily reading practice. The form is available in either English or Spanish.

The form includes space for the student's name, ZPD (Zone of Proximal Development) range, quiz title, book level, possible points, fiction/nonfiction classification, date, number of pages read in class, number of pages read out of class, total number of pages read, the percent correct on the quiz, and teacher's initials and notes.

There is also a version of this form that allows students to record their progress.
