Student Reading Log - Beginning Reader

The Student Reading Log - Beginning Reader helps you monitor student's daily reading and helps students become accountable for their own practice. This form is one of the resources available to help you implement Accelerated Reader in your classroom. You can find this form under Forms and Charts on the Resources page.

Your beginning readers (with the help of teachers, parents, or others) can use this form to record the daily reading they do in and out of class. Teachers can use it to monitor their students' daily reading practice. The form is available in either English or Spanish.

The form includes space for the student's name, ZPD (Zone of Proximal Development) for books Read To, Read With, and Read Independently, date, quiz title, book level, whether Read To, Read With, or Read Independently (TWI), monitor's initials, the percent correct on the quiz, and teacher comments.
